NDIS Plan Management: Our Plan Management team provide financial management of your NDIS Plan and are here to support you to understand your NDIS Plan. We will provide personalised support and simplify your NDIS journey.

Direct Support:  Bravo Facilitators work with you to identifiy the supports you require, and then match you with highly trained support workers. We provide supports that are safe and reliable, personalised and high quality. Some of the activities our Support Workers can assist with include:

• Supports within the home - Daily living and self-care.
• Supports within the community - Social and Community participation and Skill Development

Community Engagement and Connection Activities

• Monthly Games nights
• Group walking twice a week
• Monthly Ten Pin Bowling
